To Help You On Your Journey...
Concerned Care Hospice provides bereavement services for 13 months following the death of a hospice patient.
A staff of trained professionals provides support to individuals and families that have lost a loved one who has been in our hospice care.
The Bereavement team consists of the Bereavement Coordinator, Chaplains, Social Workers and Volunteers.
